This PR fixes #664.
Problem was that nested footnote references weren't being properly processed, as recent footnote related PRs have changed how we handle the footnote IDs. Some hashes were being left in place that shouldn't be.
This PR changes the footnote processing stage so that it always unhashes footnote hashes in the footnote body.
