Improved readme and added documentation for form field name and id attributes.
Prevented default event from firing on choose all / clear all links.
Removed Bootstrap halfling icons, jquery and bootstrap files as most people will probably leverage a CDN for those resources. Removed un-necessary quotes around image URLs in the CSS.

Hello. Awesome plugin! I added name and id attributes to the form elements, providing default values or allowing overrides from the settings dictionary.
