Per-operation context + initial payload

This pull request adds two features:

1. e86c5fe Per-operation context - allows sharing any data between different calls to Subscription.publish for the same subscription (and the same connection).
2. 39d61be Initial payload - allows sending an arbitrary "initial" response to a client as soon as it is subscribed (before any call to Subscription.broadcast).

I use them like so (in schema.py):

I've been looking for a way to have an initial payload, but am sad to see this pull request has been closed without any response from the maintainer(s).

I think that this library is dying and I need to look for or write something else that will work with graphene 3. unfortunately.
