Create feature-focused milestone epics for ge-hch.5.16 (Runtime Integration & Hooks).

What this PR does:

• Adds seven milestone epics under ge-hch.5.16 outlining implementation work: Hook Manager, Integration State Machine, Checkpointing & Rollback, Save/Load Persistence, Audit/Telemetry, QA/Fuzzing & E2E, and Docs/Runbook.
• Assigns each milestone to Build and records Success Criteria, Deliverables, and Open Questions for confirmatory review.

Created beads (IDs):

• ge-hch.5.16.3 — Hook Manager & Integration Points
• ge-hch.5.16.4 — Integration State Machine (12 states)
• ge-hch.5.16.5 — Checkpointing & Rollback Engine
• ge-hch.5.16.6 — Save/Load Persistence Integration
• ge-hch.5.16.7 — Audit Logging & Telemetry Hooks
• ge-hch.5.16.8 — QA, Fuzzing & E2E Tests
• ge-hch.5.16.9 — Docs, Runbook & Handoff

Open Questions (recorded in each bead):

• Hook API shape (async Promise-based recommended)
• Storage backend choice for checkpoints (IndexedDB recommended)
• Resume payload size limit for save files
• Integration log retention and access model
• CI fuzzing cadence and resource budget
• Runbook owner (Build by default; confirm if different)
