Notifications

AXElements used to have support for Accessibility Notifications. However, it was removed since it was a pain to maintain, it exposed some MacRuby crashes that have not been fixed, and Apple API for notifications was poorly designed.

While it is less efficient to use polling, it has shown a lot more promise for a lot less effort so far.
