Bootstrap (handshake) when starting a shell

At present, we have to submit at least one prompt to kick off the handshake sequence before the prompt can be sent to the agent. This often forces users to send a "hello" prompt just so the session can be bootstrapped and thus enable further features like changing model, mode, or resuming sessions.

This feature request tracks the work for bootstrapping sessions on startup.
