Add guides for Issues and GitHub etiquette #34

This is inspired by #30 :
one thing I'd recommend as additional topics are Issues and GitHub etiquette -- what are issues for, how to leverage issues to manage your work, and some general guidelines to interact with others on GitHub (provide enough info when asking a question, say thank you when someone helped you, acknowledge sources when mentioning something, etc.)
