Voter signup form

When user is redirected back from OpenID/OAuth a form asking user name and email should be shown for users signing in for the first time. When form is submitted, registration id should be generated using UUID. To do this, you can use [ACCOUNT_SIGNUP_FORM_CLASS](http://django-allauth.readthedocs.org/en/latest/configuration.html).
