Does not work with current VS Code (1.64)

This repo is referenced from VS Code documentation as a sample showing how to implement debugging in a notebook. However it doesn't seem to have been updated to adopt significant changes to the notebook APIs, for example in 1.56 (April 2021) and 1.57 where APIs were finalized.

This lack of a functional debugging sample isn't good.
