Add non-denominational (financially-speaking) payment options to join.html

Currently there are automated scripts to pay specified amounts via PayPal and Bitcoin.  We need unspecified options for courses.  

I tried adding `<option value="Enjigo Course">Course Fees</option>` for PayPal, but that errors out.
