FWIW, using a level 1 or level 2 header doesn't have this problem (i.e. -ag1o5 or -ag2o5).

That LHA file seems to be actually created by Unix LHA and it seems to be level 2. Level 2 files with the directory name extension use \xFF as the separator, so this needs to be handled like here: https://git.nil.im/ObjFW/ObjFW/src/commit/d19759119adf92bd295c40822586af66148eb704/src/OFLHAArchiveEntry.m#L83-L85
