Why PhpSlim Test Case giving Could not invoke constructor for MyFixture[0]

When I tried to run the Test Case by following your example. It is showing the Errors marked in Yellow color in fitnesse, when I hit the Test button.
I have already added MyFixture.php in /project_path/FitnesseRoot/Slim folder as suggested in the example.

Any Suggestion will be very helpful.

Could it be the problem of #8 ?

That is probably the wrong path. Under FitNesseRoot, you should not put your PHP code.
How does the exact wiki test look like? How did you configure the COMMAND_PATTERN?
Please share the Command in the Execution Log.
