Inviting users seems very broken

I tried inviting another of our mentors to test this out. They did not get to the invite before it expired. Upon clicking "resend", it made a new invitation. The original invitation shows "cancelled" and the new one shows "accepted". The resend and cancel buttons are on both of these (Shouldn't be able to cancel a cancelled one or resend/cancel an accepted one and none of them should be under "pending"), but that's just state bugs. I also got a lot of "could not fetch organization" errors when trying to cancel or resend these at this point.

When the mentor tried to accept the re-sent invite, they got "We couldn't accept this invitation - invitation not found". However now on my organization page, they show up as 6 rows, all with the same name and user id and such. I tried removing some of the entries from the team and they're still on the page, but it says "member not found" when I try to remove them. Additionally, it looks like on their side, they can see the organization, but it shows up as 4 separate organizations in their org dropdown.

Looking into this- does this look like what happened?

• You invited them
• They accepted the invitation
• They opened the invitation again and got "We couldn't accept this invitation - invitation not found" (this is what opening an already accepted invitation looks like)
• You sent a new invitation
• Same thing happened?
  This might also have been exacerbated by a bug I just fixed that made it look like invitations failed to accept even when they actually succeeded

Closer to:

• You invited them
• The invitation lapsed before they accepted again and they couldn't accept that invitation
• I clicked "resend" and ended up with two invitations in the list for them
• They tried accepting the second invitation but it said it failed (Assuming once, possibly more than once, didn't get a lot of clarity there as they weren't consciously tracking it for a bug report)
• They now showed up on the list 6 times with both invite rows still being there.

I suspect the bug you fixed might have been the reason them accepting the second valid invitation looked like it failed, however it wouldn't explain the duplicate invited rows and the duplicate rows in my organization. It also wouldn't explain why clicking the "remove" button fails with a notice that they're not actually on my team even though they're listed on the team and have access.
