Tiered Approval Modes (Suggest/Auto-Edit/Full-Auto)

Problem

Current sandbox is binary. Enterprise needs granular control like Codex CLI's three approval modes.

Solution

1. Suggest: Show commands only, never execute
2. Auto-Edit: Modify files, confirm before commands
3. Full-Auto: Execute with minimal prompts

cortex config set approval-mode [suggest|auto-edit|full-auto]
