Ping providers should open Github issues for failing chains

NOTE: we probably don't want to create duplicate issues for a given chain, so the title (or something unique) should be used and follow a consistent pattern. Cases for failing chains:

1. The chain has an open duplicate issue already - do nothing
2. The chain has a closed duplicate issue already - re-open it
3. The chain has no issue - create one

We may or may not want to keep posting to Slack after this is implemented
