Custom Asset Create Form

There should be the possibility of creating a new Asset using a custom form. Think of use case:
A patient visits the physician. The patient is given an IPad and asked to enter his data and describe his illness and circumstances. This practice is probably already established in US hospitals.
There should exist the possibility for the user to create forms (as a function plugin on AssetType), which are used as an input for Asset creating, after being validated either by code or person.
