Codes generated by Authentication apps do not work.

I have tried all the recommended authentication apps, but none work. Also, the single-use codes are not accepted either. The QR code gets scanned ok and added to the Authentication app; however, when trying the codes, it just comes back and says Incorrect Authorization Code. I also tried one of the auto-generated codes, which didn't even work. I could use some help with this if anyone has gotten it to work.
