option to not require a png refine_file

It would be helpful if squadgen were modified so that if no png is specified, it just spits out a template refine_map.dat w/ all zeros. And then I would suggest that when invoking the loadcsrefienmentmap flag, that it does not require a png argument, since I don't think it's used under any circumstances (?).
