idx_user_forms Block

This block must render form based on the selected form returned by the API in the block form.php by the idxApi class.

Functionality:

1. Select the form to display

2. Lead Login

3. Lead Signup

4. Login Form

5. Must provide the ability for the user to select a block view template that displays data in various formats.
   These templates will be defined at a later time.
